Ischemic heart disease is rated based on METs level and ejection fraction. The most common rating is 30% (workload of 5-7 METs resulting in dyspnea, fatigue, angina, dizziness, or syncope). The 60% requires 3-5 METs or ejection fraction of 30-50%.

Pro Tip: Know your ejection fraction and METs level. If you haven't had a recent echocardiogram, request one. Heart disease is a common Agent Orange presumptive for Vietnam-era veterans โ€” no nexus letter needed if you served in a covered area.
