Rating Criteria
60% At least one of the following
60% Constant or near-constant systemic therapy including, but not limited to, corticosteroids, phototherapy, retinoids, biologics, photochemotherapy, psoralen with long-wave ultraviolet-A light (PUVA), or other immunosuppressive drugs required over the past 12-month period
60% Persistent documented vasculitis episodes refractory to continuous immunosuppressive therapy
30% All of the following
30% At least one of the following
30% Five or more scars that are unstable or painful
30% Requiring intermittent systemic immunosuppressive therapy for control
20% Three or four scars that are unstable or painful
10% At least one of the following
10% One or two scars that are unstable or painful
0% No more than topical therapy required over the past 12-month period and at least one of the following
